A New You -
Advancements in cosmetic surgery provide patients with more natural look

By Neal Reid

For years, people have been doing whatever it takes to look younger. Thanks to advancements in technology, refined procedures and a greater understanding of less-invasive forms of treatments, cosmetic surgeons are able to provide their patients with great results along with a more natural look.

Most of those minor adjustments involve injections, according to Dr. Robert Aycock.

“The most common thing anybody does is anything that’s injectable,” said Dr. Aycock, who has offices in Walnut Creek and Marin. “For plastic surgeons and dermatologists, (the most popular are) going to be anything injectible, whether it’s botox or whatever. Other injectibles like fillers are popular to fill your face back up. As you age, you lose volume in your face.”

“In my practice, the second-most common thing would be lasers,” said Dr. Aycock. “A photo facial is the most common thing we do, and that’s where you come in, have it done and walk out and nobody knows you had anything done. We also do hair removal and resurfacing for wrinkles, and we do procedures with lasers to tighten the skin.”

Technological advancements have streamlined the laser surgery process in recent years.

“The lasers we use now compared to the lasers we used in the 1990s, people recover in just a few days instead of six months,” Dr. Aycock said. “It’s really changed.”

Preventative maintenance is important for people who want to remain looking natural while avoiding major overhauls in their later years.

“Eighty percent of the face lifts I do are for people from age 45-60,” Dr. Aycock said. “They come in and want something done before things get bad, so they beat it to the punch with a preemptive strike. If somebody comes in when they’re 70 and has a full face lift done, they’re going to look different. Whereas, if someone comes in when they’re 50, they’re not going to look different, but are just going to look better.”

Contour Turnable Resurfacing Laser Safely Erases Lip Lines

By Sean McKinney, Contributing Editor

By independently varying the ablation depth and amount of coagulation, the Contour TRL (Tunable Resurfacing Laser) from Sciton (Palo Alto, Calif.) erases lip lines without causing significant thermal damage. This dual-mode erbium:YAG laser provides a breakthrough solution to upper perioral rhytides, the greatest challenge associated with aging faces.

“The biggest benefit of this laser is that it is tunable,” said Robert Aycock, M.D., F.A.C.S., a plastic surgeon with practices in Walnut Creek and Greenbrae, California. “I have control over the amounts of ablation (removal of tissue) and heat transfer. With CO2 there is only ablation with heat transfer and the depth of ablation determines the amount of heat transfer. If you ablate deep enough with CO2 to remove lines, the transferred heat can be too great, causing loss of color, delayed healing and thin, shiny skin.”

According to Dr. Aycock, by using the Contour TRL practitioners can ablate lines with little to no heat transfer. “We ablate lines up to 800 microns without injury to the melanocytes,” he said. “We take down the ridges on either side of the line to a depth of 200 microns and then treat the upper lip surface to a depth of up to 600 microns, depending on the rhytid and thickness of the skin. Usually, the rhytides are not that deep and the skin is not that thick, so we treat more conservatively. Most treatments are at 200 to 300 microns – three times with six week intervals.”

“My approach depends on depth and degree of wrinkles – using 800 microns or more if needed,” said Jason Pozner, M.D., F.A.C.S., medical director of Sanctuary Plastic Surgery in Boca Raton, Fla., “The Contour TRL system provides more control and more predictable results than those produced by other devices.”

With decoupled ablation and coagulation effects, practitioners can use the Contour TRL to dial in the exact tissue effect needed for each patient’s pathology, desired outcome and tolerance for downtime. Erbium has much higher absorption in the target chromophore water than C02, allowing for tissue to be vaporized with little thermal conduction.

“The downtime associated is less with pure ablation because there is little thermal injury to the skin and less erythema or redness,” confirmed Dr. Aycock, who uses only local anesthetics for treatment.

The dual mode erbium Contour TRL can create the same tissue effect and histology as CO2 and yttrium scandium gallium garnet (YSGG) lasers. “Skin does not know the difference among erbium, CO2 and YSGG,” Dr. Aycock explained. “All that applies are ablation and heat. With the Contour TRL, the erbium is pure ablation when the power is at 1 J/cm2 or above. We can add heat with subablative pulses (power of less than 1 J/cm2) or dial out heat and use only ablative pulses.”

Dr. Aycock and Dr. Pozner, who use only pure ablation for upper lips, are scheduling more treatments because of the Contour TRL. “The laser offers more options,” said Dr. Aycock, who also combines the Contour TRL with dermal fillers, neuromodulators, SkinTyte and BBL BroadBand Light therapy (also from Sciton).

“I use the Contour TRL with other surgeries and as a surgical alternative for lower lids,” added Dr. Pozner. “This laser meets many patient needs and expands our offerings.”

About Face

When performing facial rejuvenation surgery, to achieve natural-looking results, Dr. Aycock asserts that it’s imperative
to use COMBINATION TREATMENTS that address multiple signs of aging,” he says, noting that different regions of
the face present unique challenges. “Some patients need more work around the eyes, some require treatment in the
mid-face, while others will really benefit from a browlift,” he explains. “Our goal is to give each patient what they need
to look their best.” Dr. Aycock routinely draws from a TOOLBOX OF ANTI-AGING PROCEDURES to turn back time.
For example, fat transfer can help PLUMP SUNKEN FEATURES, while lasers can remove unwanted hair, erase age
spots and TONE SLACK SKIN. “Sometimes a patient might only need fillers to obtain his or her desired look,” says

Complete Woman Magazine – May / June 2008

Your Skin at 20, 30, 40, 50 plus

It’s never too late to start pampering your skin, says San Francisco Bay Area plastic surgeon Robert Aycock. At this stage, topical treatments – combined with a healthy lifestyle – still have plenty of wrinkle-fighting promise. However, they can’t replace doctor’s office procedures when it comes to eliminating serious sun damage and sagging.

“There’s a big difference between skin at 45 and 55,” Dr. Aycock says. “In your 50′s, you lose fat in the face and things start to slide downward, which is why some women opt for a facelift.” During menopause, skin continues to thin and becomes more sensitive to climate changes. You’re likely to experience more brown spots and facial hair (although eyebrows become sparse – go figure). Bags or hollows around the eyes are more pronounced.

Keep your face from matching your leather handbag by reaching for a creamy cleanser and richer moisturizer, and using exfoliants more often to slough off dead skin (think ferulic acid, glycolic acid, and retinols). Your beauty mantra? Moisturize, moisturize, moisturize.

Living in the same county as his surgical center, which is fully
equipped with overnight recovery accommodations, enables Dr.
Aycock to take comfort in knowing he is easily accessible should his
patients need him. “All surgical patients are educated in-depth prior
to procedures and receive follow-up calls after surgery,” he says of
his efforts to make his patients’ experience as pleasant and smooth
as possible. “I spend a great deal of time getting a true sense of
each patient, as well as his or her objectives. It is only then, when
neither one of us has any more questions, that I will proceed
with treatment.”

emphasis on efficiency and

CUSTOMIZED FACIAL REJUVENATION

Believing that a personalized treatment plan will help yield what he describes as “YOUTHFUL RESULTS THATWORK
TO COMPLEMENT YOUR NATURAL FEATURES,” Dr. Aycock and his tenured staff spend ample time understanding
patients’ goals and obtaining a thorough appreciation for the type of results they hope to achieve. “Prior to seeing me,
patients will receive a detailed questionnaire in the mail to ensure our staff is aware of any special needs or
accommodations they may require,” says Dr. Aycock. “To further make my consults more efficient, patients first
meet with my patient care coordinator, who will enlighten me as to the concerns they would like to discuss. This
enables me to make MORE PRECISE RECOMMENDATIONS in relation to their specific needs.” To help patients look
younger, Dr. Aycock offers a wide variety of surgical and nonsurgical services. Such treatments include FACELIFTS,
EYELID LIFTS, BROWLIFTS, FACIAL IMPLANTS AND INJECTABLES. “Sometimes a patient will benefit most from
combination therapies; other times a patient might only need fillers to obtain his or her desired look. I think patients
reap the most advantages when they receive a customized treatment plan that takes balance, harmony and
proportion into account. There are multiple treatments and techniques to use that can be specifically tailored to each
patient and result in limited incisions, as well as shorter procedures.”

Get Ready to Party

Settle in for a cleansing facial. Beauty connoisseurs Madonna and Eva Longoria are hooked on oxygen therapy facials. No need to make the jaunt to Hollywood – Julie Levesque gives the treatment inside the Walnut Creek office of Plastic Surgeon Robert Aycock. After a quick cleanse, she spends thirty minutes waving a metal, air brush-like wand directly on top of the face. Hyperbaric oxygen infuses a plumping serum (made up of hyaluronic acid, green tea, and Vitamins A, C, and E) into the skin. Like dipping your face into the fountain of youth, it diminishes expression lines, and leaves your skin looking noticeably quenched. Even the lips get a slight boost in size. If you want to measure the results, ask Levesque to infuse one side of your face at a time and compare the “before” with the dewy, supple “after.”
