Contour Turnable Resurfacing Laser Safely Erases Lip Lines
By Sean McKinney, Contributing EditorBy independently varying the ablation depth and amount of coagulation, the Contour TRL (Tunable Resurfacing Laser) from Sciton (Palo Alto, Calif.) erases lip lines without causing significant thermal damage. This dual-mode erbium:YAG laser provides a breakthrough solution to upper perioral rhytides, the greatest challenge associated with aging faces.
"The biggest benefit of this laser is that it is tunable," said Robert Aycock, M.D., F.A.C.S., a plastic surgeon with practices in Walnut Creek and Greenbrae, California. "I have control over the amounts of ablation (removal of tissue) and heat transfer. With CO2 there is only ablation with heat transfer and the depth of ablation determines the amount of heat transfer. If you ablate deep enough with CO2 to remove lines, the transferred heat can be too great, causing loss of color, delayed healing and thin, shiny skin."
According to Dr. Aycock, by using the Contour TRL practitioners can ablate lines with little to no heat transfer. "We ablate lines up to 800 microns without injury to the melanocytes," he said. "We take down the ridges on either side of the line to a depth of 200 microns and then treat the upper lip surface to a depth of up to 600 microns, depending on the rhytid and thickness of the skin. Usually, the rhytides are not that deep and the skin is not that thick, so we treat more conservatively. Most treatments are at 200 to 300 microns - three times with six week intervals."
"My approach depends on depth and degree of wrinkles - using 800 microns or more if needed," said Jason Pozner, M.D., F.A.C.S., medical director of Sanctuary Plastic Surgery in Boca Raton, Fla., "The Contour TRL system provides more control and more predictable results than those produced by other devices."
With decoupled ablation and coagulation effects, practitioners can use the Contour TRL to dial in the exact tissue effect needed for each patient's pathology, desired outcome and tolerance for downtime. Erbium has much higher absorption in the target chromophore water than C02, allowing for tissue to be vaporized with little thermal conduction.
"The downtime associated is less with pure ablation because there is little thermal injury to the skin and less erythema or redness," confirmed Dr. Aycock, who uses only local anesthetics for treatment.
The dual mode erbium Contour TRL can create the same tissue effect and histology as CO2 and yttrium scandium gallium garnet (YSGG) lasers. "Skin does not know the difference among erbium, CO2 and YSGG," Dr. Aycock explained. "All that applies are ablation and heat. With the Contour TRL, the erbium is pure ablation when the power is at 1 J/cm2 or above. We can add heat with subablative pulses (power of less than 1 J/cm2) or dial out heat and use only ablative pulses."
Dr. Aycock and Dr. Pozner, who use only pure ablation for upper lips, are scheduling more treatments because of the Contour TRL. "The laser offers more options," said Dr. Aycock, who also combines the Contour TRL with dermal fillers, neuromodulators, SkinTyte and BBL BroadBand Light therapy (also from Sciton).
"I use the Contour TRL with other surgeries and as a surgical alternative for lower lids," added Dr. Pozner. "This laser meets many patient needs and expands our offerings."
